Printed PEOS Certificate
Printed E-Registration Certificate
Additional Requirements for Ex- Taiwan
Old Passport (with entry & exit Visa)
ARC/ Health Insurance card
Old Passport was lost
Certificate of Entry and Exit from Bureau of Immigration
Notarized Affidavit of Loss
